The application file must include the following documents: A duly completed and signed academic leave application form, according to the approved template. A copy of the enrollment or re-enrollment certificate for the 2026-2027 academic year. Supporting documentation justifying the academic leave request. In the case of medical justification, the document must be endorsed by the university physician.
